School registration checklist
Kindergarten Enrollment Eligibility:
All children residing in the City of Chicago who are 5 years old on or before September 1 of the current school year, are eligible for enrollment into a CPS Kindergarten program.
The parent, legal guardian, or temporary custodian of any elementary age child enrolling in a Chicago public school is required to submit proof of age and required medical records to the school.
First Grade Enrollment
Students must be six years old by September 1, 2017, in order to apply for first grade enrollment for the 2017-2018 school year. However, if your child will become six years old between September 2, 2017 - December 31, 2017, he/she may qualify for an exception to the age requirement for first grade enrollment.
To qualify for this exception, your child:
Must become six years old between September 2, 2017 - December 31, 2017
If you are applying for first grade enrollment for your child under this age exception, you must apply via paper application and you must submit a First Grade Age Exception Form to the Office of Access and Enrollment along with your application.

Dental requirement
All students in Kindergarten, 2nd, and 6th grade must have a dental exam completed by a licensed dentist prior to May 15th of the current school year.
Health Requirements:
To complete your child’s registration you must provide the health documents required by CPS to the school office no later than October 15th, 2017
If you have questions about what your child needs for school, please call your school's main office. Schools will help identify what your child needs to be in medical compliance. Also, ALL students must turn in the Student Medical Information Form to their schools at the beginning of the year.

- Must have attended preschool and kindergarten at the same nonpublic school, or the same public school outside Illinois
- Must have been taught in kindergarten by an appropriately certified teacher
- Must be deemed ready by the nonpublic school (or the public school outside Illinois) to work at the first grade level

- Current utility bill
- Illinois driver's license or State of Illinois identification card
- Deed
- Employee identification number
- MediPlan/Medicaid card
- Court documents
- Illinois Department of Public Aid card
- Stamped United States Post Office change of address form
- Illinois state aid check/social security check
